Clinic location, hours

The FDP Granite Clinic is located at 2500 S. State, Suite 404. It is open from 7:30 a.m. to 6 p.m. Monday through Thursday and sees patients on Medicaid, CHIP and PCN. For more information or an appointment, call 801-646-4377.
